Authorization For Repairs and Payment Policy

I, the undersigned, hereby authorize Huber Collision and its employees to provide an estimate to repair my vehicle and, unless the vehicle is declared a total loss, to perform the identified services required to repair the vehicle. I understand it may be necessary to drive my vehicle for the purpose of testing and/or inspection.

I understand that an express mechanic's lien is acknowledged on the vehicle to secure the amount of repairs thereto.

I understand that Huber Collision will NOT be held responsible for theft of vehicle or missing articles left in vehicle such as: ipods, ipads, cameras, sunglasses, credit cards, money, CD's, golf clubs, and etc. Please remove all items from your vehicle before or when you drop off your vehicle for repairs.

I understand that Huber Collision will NOT be responsible for any auto rental during the repair process.

I understand that Huber Collision will impose a $85.00 per day storage fee after 48 hours from my notification of the completion of the vehicle repair. I further understand that the storage fee policy is handled on a case-by-case basis when the vehicle is declared a total loss by the insurance company.

I further understand that Huber Collision is unable to release any vehicle without full payment.

I am responsible for payment in full for the repairs of my vehicle either before or after completion of work, but prior to picking up the vehicle. It is my responsibility to forward all payments I receive from the Insurance Company or from the party whom is paying for the repairs directly to Huber Collision for said repairs.

I authorize the Insurance Company to pay Huber Collision direct for all original and supplemental repairs. I authorize Huber Collision to endorse my name to any checks received from the Insurance Company or from the party whom is paying for the repairs of my vehicle.

I understand that Huber Collision cannot accept any personal check in excess of $250.00.
